1. Program Period: 1 June to 31 August 2024
2. Eligibility for the Program: The first 200 customers who sign up for at least 4 cycles of CoolSculpting or CoolSculpting Elite treatment* (in one transaction) with participating medical clinics in Bangkok during the Program Period will be eligible to participate in the Program. Each customer is only entitled to one participation chance in the Program.

   * Number of treatment cycles is subject to medical professional’s evaluation and recommendation of what is appropriate for the patient. The medical professional will create a personalized treatment plan tailored to the patient’s goals.

3. Participation in the Program:
   • An eligible customer shall register for the Program by scanning the Program’s QR code at a participating medical clinic, and uploading the relevant participating clinic receipt/slip in the Program website via the QR code link.
   • All uploaded receipts/slips must contain the following details:
     • Full name of the participant
     • Clinic name; and
     • Description of treatment/services (showing at least 4 cycles of CoolSculpting cycles).

   Only legible and clear receipts/slips shall be accepted for review for eligibility for the Program; all illegible or blurred receipts/slips shall automatically be disqualified.

   • Upon successful registration and verification, a coupon code for 5,000 THB voucher will be sent via SMS (from CScampaign) to the participant. This voucher may be used towards the participant’s next CoolSculpting treatment at any participating medical clinic by 31 August 2024.
   • List of participating medical clinics: Apex Medical Center, Romrawin Clinic, Dermaster Beauty Clinic, Hertitude Clinic, Doctor Yada Clinic
   • Use of voucher: When a successful participant wishes to redeem the voucher for his/her next CoolSculpting treatment, the participant shall show the voucher code to a participating medical clinic, and the clinic staff will confirm the redemption of the voucher (via an online form). The medical clinic will set off the voucher against the participant's bill for CoolSculpting treatment. For the avoidance of doubt, there will be NO direct reimbursement or payment given to the participant.
4. Allergan Aesthetics is not responsible for any problems or technical malfunction of any phone network, computer online systems, servers, or providers, computer equipment, software, technical problems or traffic congestion on the Internet or at any website, or any combination thereof, (including but not limited to) any injury or damage to participants or any other person’s computer, related to or resulting from participation in the Program.
5. If the Program cannot run as planned, including, but not limited to, infection by computer virus, bugs, tampering, unauthorized intervention, fraud, technical failures, prohibition by the relevant authorities or any other causes beyond the control of Allergan Aesthetics which corrupts or affect the administration security, fairness, integrity or proper conduct of this Program, Allergan Aesthetics reserves the right in its sole discretion to disqualify any individual who tampers with the entry process, and/or to cancel, terminate, modify or suspend the Program as it sees fit.
6. Allergan Aesthetics shall not be liable for any injury, loss or damage whatsoever which is suffered (including, without limitation, indirect or consequential) as a result of taking part in the Program, except for any liability which cannot be excluded by law.
7. All personal details of the participants collected by Allergan Aesthetics or its representative managing the Program shall only be used for the purpose of administering the Program. Such personal details shall be dealt with in accordance with the privacy policy terms
8. The Program is valid from 1 June 2024 until 31 August 2024, or until all vouchers are redeemed, whichever is earlier.
9. Allergan Aesthetics reserves the right to modify the terms and conditions of this Program at any time without prior notice.
10. The laws of Thailand shall govern the terms and conditions of this Program.

1. Data Collection. We will collect the following information from you through our online registration system: first name, last name, mobile phone number, and clinic/organization details. This information is necessary to verify the identity and details of activity participants.
2. Purpose of Collection. The information received will be used solely for the purposes of administering the special discount activity, including contacting you, confirming registration details, verifying eligibility, issuing discount codes, internal management purposes, and compliance with legal requirements.
3. Data Disclosure. Your personal data will not be disclosed or shared with any external parties, except when required by government orders or laws, or with your consent.
4. Data Security. The Company employs appropriate security measures to prevent loss, unauthorized access, use, modification, or disclosure of data. This includes access controls, data encryption, and security audits.
5. Data Retention and Destruction. Your personal data will be retained only for as long as necessary for this activity and the stated purposes in this policy. After 30 days from the end of the activity, all data will be permanently destroyed, unless required by law to be retained further.
6. Data Subject Rights. Under the Personal Data Protection Act, you have the right to access, rectify, erase, transfer, or object to the processing of your personal data, as well as lodge complaints in case of non-compliance. You can contact the Company's Data Protection Officer through the prescribed channels.
7. Policy Changes. The Company may amend this policy from time to time, with prior notification through appropriate channels before the changes take effect.
